[B] Scope of Service:
(1) Vendor needs to provide youth employment program data system for create one database with each agency only allowed to access its own participants and document in real time all the daily activities of the programs, able to have access to the entire database and use it to track in real time the activities of each agency in achieving contract performance, secure and have layered security protocols for protection of private information, allow application entry, participant enrollment and worksite application entry to operate one cycle at a time and keep the information from each cycle independent from other cycles, allow application entry, participant enrollment, and worksite application entry, customizable, creating intake forms for applicants with data validation based on specifications to improve quality and accuracy of participant data, ability for an agency to create a secondary intake repository, allow applicants to save in-progress applications, receive notifications regarding application status via email or text message, create unique login and password for each youth applicant, allow provider/employer to search for complete applications, allow providers or employers to log in to track weekly participation, enter evaluation information, and enter various program data, track program participation hours, create and send messages from the administrator to all vendor agencies and fill in and print out program reporting forms, maintain a database of all participant and worksite records and create ad-hoc reports as needed in addition to regular reports.
(2) All questions must be submitted in writing no later than January 28, 2019.

[D] Work Performance:
Performance of the work will be Offsite. Vendor needs to carry work in their office location.
