(1) Vendor needs to provide annual physical and entrance physical exam services to the government authority located in Providence, RI.– Perform all examinations and testing, and provide written reports (including interpretations, evaluations, and recommendations) to the individual members' home address within ten working days of examination.– Comprehensive occupational and medical history;– Physical assessment– Complete binocular vision examination (color, near distance, depth peripheral};– Total cholesterol, HDL, LDL– Tb screening– Tdap vaccine (every 10 years)– Heavy metals blood– HIV screening(2) All the questions must be submitted due on than July 5, 2024.
