[A] Budget: Looking for proposal
[B] Scope of Service:
(1) Vendor needs to provide community referral platform must be able to enhance service provision, and over time, needs to be able to become well integrated into the workflows of Platform users including and especially the workflow of health care providers.
- It must have a flexible architecture to facilitate statewide adoption and allow for interfaces and integration to native technology systems including electronic health records
- Finally, its data model and architecture must allow it to provide with data to better understand how to optimize health and well-being for all people by effectively stewarding resources that bridge our communities and our healthcare system, and the social needs captured by the screens and referrals in the system
- The state envisions a fully functional comprehensive web-based system as the final software deliverable
- The state is seeking a vendor to help accomplish its goals as part of the Medicaid’s comprehensive accountable entity program to addressing the health-related social needs of its beneficiaries by establishing and maintaining a community resource platform
(2) A pre-bid meeting will be held on December 22, 2020.
(3) All question must be submitted no later than January 4, 2021.
[C] Eligibility:
- Onshore (USA Organization Only);
[D] Work Performance:
Performance of the work will be Offsite. Vendor needs to carry work in their office location.
