[A] Budget: Looking for Proposal
[B] Scope of Service:
(1) Vendor needs to provide data conversion and encryption services to include data conversion - tape to CD, price per original CD, data conversion - price per CDR page, data conversion miscellaneous - price per CDR page and CD/DVD's produced will have 256-bit encryption.
- Data Conversion - taking reports from the FARS application (business affairs application), the charge bill application (benefit charge), and the tax application (financial report) and putting the data files on CD, creating reports, FARS report indexed by ledger number and report name, charge bill report indexed by EIN number, financial audit report indexed by EIN number.
- The volume is approximate is FARS - 20000 pages, separated into daily and monthly reports, benefit charge - 5500 pages, separated into daily and monthly reports and tax report - 3000 pages.
(2) All question must be submitted no later than April 26, 2019.

[D] Work Performance:
Performance of the work will be Offsite. Vendor needs to carry work in their office location
